About LucidCore Health
LucidCore Health is a Management Services Organization (MSO) that provides health and wellness education, intake and evaluation services and connects customers with an independent licensed clinician.
LucidCore Health is not a medical practice and does not provide medical services.
LucidCore does not diagnose medical conditions, prescribe medications, sell or dispense medications, or make decisions regarding medical treatment.
All medical decisions are made by licensed clinicians. Treatment occurs through the clinician's platform.
Consultation & Evaluation
LucidCore offers a Consultation & Evaluation for $210.
The evaluation is intended to help collect information regarding your health and wellness goals and facilitate your connection with an independent licensed clinician.
Purchasing a LucidCore Consultation & Evaluation does not guarantee that you will qualify for or receive any medical treatment.
The licensed clinician independently determines whether treatment is medically appropriate.
Clinician Relationship
After purchasing your Consultation & Evaluation, you will be redirected to the clinician's platform to schedule your medical consultation.
The clinician is independently responsible for all medical evaluation, diagnosis, prescribing, treatment and follow-up care.
LucidCore does not direct or control the clinician's professional medical judgment.

Payment and Refunds
The fee for the LucidCore Consultation & Evaluation is $210.
